                                                                 April 16, 2009

What Is National Healthcare Decisions Day?

The National Healthcare Decisions Day (NHDD) initiative began in 2008 as a collaborative effort of national, state, and community organizations committed to ensuring that all adults with decision-making capacity have the information and opportunity to communicate and document their heallthcare decisions.  On April 16, 2009, NHDD events will be held at hundreds of locations across the country.  Minnesota event attendees will have the opportunity to learn more about and complete a Health Care Directive.  About NHDD

What Is A Health Care Directive?

A Minnesota Health Care Directive is a document through which individuals may exercise their right to control their own health care in the event that they are not able to speak for themselves.  Under Minnesota law passed in 1998, a Health Care Directive combines two separate documents: a  Living Will and a Durable Power of Attorney.  Through a Minnesota Health Care Directive, individuals may name an agent to act on their behalf when unable to speak for themselves (Durable Power of Attorney for Health Care), provide specific health care preferences and instructions (Living Will), or both.   Individuals will have access to further information and will be able to complete a Minnesota Health Care Directive at most NHDD events.  Sample Minnesota Health Care Directive.
